Survey for UX consultants (to help new consultants get started)

Hello folks,
I am co-running a workshop this month on how to set yourself up as a consultant.  I'd like to present broader data than just my own and my co-presenter's experiences.   It would help me tremendously to have consultants fill out our survey on their basic setup:  what they offer, why they consult, how they get their work, what rates they set, etc.
