I was looking for an online survey too... i´ve found 2 in php that
looks interesting but i think that it wont do everything i need...

I'm lost (as usual). How do you intend to use survey software to collect
user workflow - do you want people to report their task sequence in a
survey tool? That said, what attributes would specifically relate to
user workflow that would differ one survey package to another. I have
only worked with a few, but have always felt that the challenge is
tweaking a particular survey instrument to the strengths & limitations
of a tool.

Genuinely, I'm not sure what I'm missing here ;) Oh, were you talking
about analytics software - something that counts & measures paths
through a site or app?

For a graduate project, I created an online survey tool call Survey Studio. It does not have the reporting features you want, but in the process I analyzed a few other apps that might:

-Survey Monkey - overall the easiest-to-use application (though a couple interfaces take some learning). Pretty good reporting tools, and widely used by people in industry.
-Key Survey - Good combination of ease of use and functionality
-Question Pro - lots of features but terrible interaction

For each of these you may have to pay a monthly fee to get the features you want.

Haven't used it to create a survey but I recently participated in a
37signals survey that was made with SurveyMonkey. It seems that you can
totally skin it to your branding need. They let's you see results as bars,
don't know about pies, points etc.
