UIs for Uploading Images

First, thanks to everyone who responded to my question about UIs for
creating fill-in forms. This is a great group!

I'm now looking for examples of UIs for uploading images (such as
photos or logos) to a web site. There are of course many out there;
are there any you think are especially good?

UIs for Creating Forms

I'm looking for examples of UIs that allow people to create their own
fill-in forms or modify existing forms. One that I've used is SurveyMonkey

Are there others that are especially good? Are there any that allow

open source contributors needed

I am looking for responses from a larger number of participants who
have contributed to an open source project where usability is
considered.

As a graduate student in the College of IST at Penn State University,
I am researching the role of usability expertise in Free/Libre/Open
source Software (FLOSS) projects. This includes looking at how
usability issues are handled.

Definitions of User Experience..

I'm hoping to collect some reasonably "canonical" definitions of "user
experience" and dare I suggest it, "experiences" in general.

Anyone have some good sources for these definitions?
